Patient Information

We’ll have you smiling the same day.

Deciding to transform your smile takes a lot of thought and consideration. We want to make things as easy for you as possible, so you can feel confident about the direction you’ve chosen. If you choose to move forward with treatment, rest assured that you’ll never be without a smile. You’ll walk in and out of treatment the same day with a set of fully functional teeth that fit comfortably in your mouth.

First Visit

We want you to leave your first visit feeling comfortable with our team and knowledgeable about the customized treatment plan we’ve put together for you.

Your first visit includes a personalized consultation during which we will discuss your overall health, wellness and aesthetic goals. Together, we will review your past experiences, medical history and put together a plan for you to achieve the smile of your dreams. We’ll also take a CAT scan to determine whether you are a candidate for dental implants, along with a photographic series to determine aesthetic outcome and desires, and a digital scan of your dentition to initiate optimal planning. Our team is happy when our patients are happy, and we know careful evaluation and planning is key to your satisfaction with the end result. We will take the time to get to know you, allow you to get to know us, and work together toward the end goal.

Please bring the following materials to your first visit:

  • If you have been referred, please bring your referral slip. Self-referrals are always welcome.
  • A list of medications you are currently taking.
  • Your new patient paperwork (click to download)

X-Rays

If your dentist or physician has taken X-rays, you may request that they send them to our office. We will gladly accept them in digital or physical form. We can also take new X-rays at our office.

Note: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit. Please be sure to let our team know if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.).

Accommodations

Through a partnership with Hyatt Place Louisville East we are able to offer patients who are traveling long distances a discount on overnight stays. Follow this link for Rates and Room information.

Insurance Information

At Advanced Dental Solutions, we want to give you the smile you desire, not one based on dental or medical insurance allowances. We will review the financial commitment of the treatment with you every step of the way. While some procedures may be covered by your insurance, we ask that you cover the cost of the treatment directly with our office. Then, as a courtesy, we will prepare and submit an insurance claim on your behalf. If the treatment is covered, the reimbursement from your insurance provider will be provided to you, not to our practice. We appreciate your understanding, and are always happy to answer any questions that you have. Please bring your insurance information with you to your consultation so that we can expedite any potential reimbursement.
